ENDORSED ORDER. Plaintiff, Jay Thomas Stubbs, an inmate of the Florida penal system, filed an unsigned pro se document #1 titled, "Sworn Affidavit/Urgent Matter." Stubbs asserts medical providers at Florida State Prison are unnecessarily delaying the removal of a metal object that has been in his abdomen since 2018 and are placing chemicals or substances in his food in hopes that the metal object will dissolve, to avoid surgery. He also complains of various other perceived violations, including the following: interference with his legal mail; sexual, mental, and physical harassment; destruction of his personal property; falsification or manipulation of records; and denial of proper medical treatment (unrelated to the metal object). Stubbs's filing is not a proper complaint, does not include all of the information required by the court-approved civil rights complaint form, and cannot commence a civil action. See Fed. R. Civ. P. 3 ("A civil action is commenced by filing a complaint with the court."); see also M.D. Fla. R. 6.04(a) ("A pro se person in custody must use the standard form[.]"). Accordingly, the Clerk of Court shall close this case and send Stubbs a civil rights complaint form. If Stubbs chooses to refile his claims, he may do so on the provided form.
