Filing 11 ORDER re #9 Prisoner consent form and financial certificate filed by Steven Sean Jackson. Plaintiff's Motion to Proceed In Forma Pauperis (Doc. 9) is GRANTED to the extent that the case may proceed without the prepayment of the entire filing fee. Plaintiff must complete each of the enclosed forms as follows for each Defendant: Notice of a Lawsuit and Request to Waive Service of a Summons, Waiver of the Service of Summons, and Marshal's form (Form 285). The completed forms must be returned to the Clerk's Office within FOURTEEN (14) DAYS from the date of this Order. Failure to return the completed forms within this time period will result in dismissal for failure to prosecute without further notice. The Clerk of Court is directed to mail three of each of the above mentioned forms to Plaintiff, and he must then mail the completed forms to the Clerk's Office. The Clerk of Court shall MAIL a copy of this Order to: Administrator of the Inmate Trust Fund Account, Brevard County Jail, 860 Camp Road, Cocoa, Florida, 32927. See Order.
