July 18, 2022 Opinion or Order Filing 3 ORDER denying without prejudice #2 Motion to Proceed In Forma Pauperis. Plaintiff must submit a consent form indicating his understanding that he is required to pay the full filing fee in this case, even if his motion to proceed in forma pauperis is granted. Plaintiff shall amend his complaint as described above within TWENTY-ONE (21) DAYS from the date of this Order. Plaintiff is advised that failure to do so within this time will result in the dismissal of this action without prejudice without further notice. Within TWENTY-ONE (21) DAYS from the date on this Order, Plaintiff shall either file a fully completed Prisoner Consent Form and Financial Certificate (if Plaintiff desires to proceed as a pauper) or pay the $402.00 filing fee. Plaintiff shall immediately advise the Court of any change of address. The Clerk of Court is directed to mail Plaintiff a standard prisoner civil rights complaint form and a Prisoner Consent Form and Financial Certificate along with this Order.
