ORDER denying without prejudice #2 Motion to Proceed In Forma Pauperis. Plaintiff must submit a consent form indicating his understanding that he is required to pay the full filing fee in this case, even if his motion to proceed in forma pauperis is granted. Within TWENTY-ONE (21) DAYS from the date on this Order, Plaintiff shall either file a fully completed Prisoner Consent Form and Financial Certificate (if Plaintiff desires to proceed as a pauper) or pay the $402.00 filing fee (if Plaintiff does not wish to proceed as a pauper). Plaintiff shall immediately advise the Court of any change of address. He shall entitle the paper "Notice to the Court of Change of Address" and not include any motions in it.
