Plaintiff shall file an Amended Complaint as described above within FOURTEEN (14) DAYS from the date of this Order. The Clerk of the Court shall mail Plaintiff a court-approved form to use for filing the Amended Complaint. After Plaintiff files the Amended Complaint, he may submit an amended motion to proceed in forma pauperis. Within FOURTEEN (14) DAYS from the date on this Order, Plaintiff shall either file a fully completed Prisoner Consent Form and Financial Certificate (if Plaintiff desires to proceed as a pauper) or pay the $402.00 filing fee (if Plaintiff does not wish to proceed as a pauper).
