As this case is closed, Plaintiff should not file any further motion, paper, pleading, or notice in this case other than a single motion related to reconsideration of this order of dismissal or a motion or notice related to an appeal of this order. Any other filing will be immediately stricken. The Clerk is directed to unseal this case, close this case, docket and then immediately strike any motion, paper, pleading, or notice filed by Plaintiff in this case after the entry of this order; and mail to plaintiff, along with this Order, a non-prisoner civil rights complaint form and an affidavit of indigency form..
