ORDER dismissing case without prejudice as a shotgun pleading. If Plaintiff desires to proceed with this case, he must file an amended complaint in accordance with Section III.A above within TWENTY-ONE (21) DAYS after the date of this Order. Should Plaintiff fail to timely amend his complaint, the case will be dismissed without prejudice for failure to prosecute. Plaintiff has not paid the filing fee or moved to proceed in forma pauperis. Within TWENTY-ONE (21) DAYS from the date of this Order, Plaintiff shall either file a fully completed Prisoner Consent Form and Financial Certificate (if Plaintiff desires to proceed as a pauper) or pay the $405.00 filing fee (if Plaintiff does not wish to proceed as a pauper). Failure to do so or to explain noncompliance within the allotted time may result in the dismissal of this action without further notice. Plaintiff has filed this action pro se, and he is directed that he must immediately advise the Court of any change of address. He shall entitle the paper "Notice to the Court of Change of Address." This notice shall contain only information pertaining to the address change and the effective date of such and should not contain other motions or requests. Failure to inform the Court of an address change may result in the dismissal of this case without further notice. The Clerk of Court is directed to modify the docket to reflect that Plaintiff is also known as "Jerel Adrian Perez" and is directed to mail to Plaintiff, along with this Order, a copy of the standard prisoner civil rights complaint form and the Prisoner Consent and Financial Certificate form.
