ORDER GRANTING LEAVE TO PROCEED IN FORMA PAUPERIS. Plaintiff's motion to proceed in forma pauperis (Doc. #8 ) is GRANTED to the extent Plaintiff may proceed with this case without prepaying the entire filing fee. ORDER. The Clerk of Court is directed to send Plaintiff a civil rights complaint form for use by prisoners and the Court forms for seeking leave to proceed in forma pauperis. The Clerk must include this case number on the forms. Plaintiff's "Motion for Qualification for In Forma Pauperis Status" and "Motion for Court Order Injunction" (Docs. #2 , #3 ) are DENIED as deficient and without prejudice to Plaintiff's refiling them in compliance with the directives of this order.
