ORDER - The Clerk of Court shall forward to Plaintiff an application packet for requesting leave to proceed in forma pauperis by state prisoners. Plaintiff has until 3/1/2024, to either: (1) file an amended in forma pauperis motion (which must include a signed prisoner consent form), or (2) pay the full amount ($405.00) of the filing fee. The Clerk of Court shall also provide Plaintiff with a civil rights complaint form for use by state prisoners. Plaintiff shall have until 3/1/2024, to file a civil rights complaint form if he desires to continue this case.
