January 4, 2022 Opinion or Order Filing 3 ORDER re #1 Complaint. The clerk is directed to identify this case as one filed by a prisoner, rather than a non-prisoner. The clerk shall send Plaintiff a complete set of forms for use by prisoners to (1) file a motion to proceed in forma pauperis; (2) file a civil rights complaint under 42 U.S.C. 1983, and (3) file a notice of change of address. This case number should be written on the forms and Amended Complaint shall be written on the complaint form. Within twenty-one (21) days from the date of this order, Plaintiff shall file a complete motion to proceed in forma pauperis or pay the $402.00 filing fee, file an amended complaint, and file a notice of change of address. The amended complaint shall be typed or clearly written and submitted on the provided Court form. In the interim, the clerk is directed to send a copy of this order and the forms referenced herein to Plaintiff at: Florida Womens Reception Center, 3700 NW 111th Place, Ocala, FL, 34482-1479.
