ORDER GRANTING LEAVE TO PROCEED IN FORMA PAUPERIS. Plaintiff's second motion to proceed in forma pauperis, ECF Doc. #8 , is GRANTED to the extent this case may proceed without prepayment of the entire filing fee. However, the total filing fee in this case remains $350.00. Pursuant to the Prison Litigation Reform Act of 1995, 28 U.S.C. 1915, even though Plaintiff is granted leave to proceed in forma pauperis, he must pay the entire $350.00 filing fee in full REGARDLESS OF THE DISPOSITION OF THE DISTRICT COURT CASE (including dismissal). ORDER. The clerk shall send Plaintiff a complete set of the forms required for a prisoner to file a motion to proceed in forma pauperis. This case number shall be written on all forms. The clerk shall send Plaintiff a civil-rights complaint form for use by prisoners in actions under 42 U.S.C. 1983. This case number and the words "Amended Complaint" shall be written on the form. Within twenty-one (21) days from the date of this Order, Plaintiff shall either file a completed motion to proceed in forma pauperis or pay the $402.00 filing fee. This case cannot proceed until all the required forms are submitted. Within twenty-one (21) days from the date of this Order, Plaintiff shall file either a notice of voluntary dismissal or an amended complaint. If Plaintiff files an amended complaint, it must be typed or clearly written and submitted on the appropriate form provided with this Order.
