Filing 4 ORDER. The clerk shall correct the docket entry for ECF Doc. #2 to describe it as a Prisoner Consent Form and Financial Certificate, rather than as a motion. The clerk shall send Plaintiff a copy of the Court's complete set of the forms needed to file a motion to proceed in forma pauperis for prisoners. This case number should be written on the forms. Within twenty-one (21) days from the date of this Order, Plaintiff shall either pay the full filing fee or file a complete motion to proceed in forma pauperis, which includes the following: 1) the motion; 2) an affidavit in support of the motion; 3) an executed prisoner consent form, 4) a financial certificate, completed by an authorized official; and 5) a printout of the transactions in Plaintiffs inmate trust account for the six (6) months preceding the filing of the complaint.
