The clerk shall send Brown: (1) a civil rights complaint form (for non- prisoners) in actions under 42 U.S.C. 1983; and (2) the forms needed to file an application to proceed in forma pauperis (for non-prisoners). This case number should be written on the forms and "Amended Complaint" should be written on the complaint form. Within fourteen (14) days of the date of this Order, Brown shall file either a notice of voluntary dismissal or an amended complaint. If Brown files an amended complaint, it must be typed or clearly written and submitted on the Court form as instructed above, and it must correct the deficiencies identified herein. Within fourteen (14) days of the date of this Order, Brown shall either pay the $405 filing fee or file a complete application to proceed in forma pauperis on the required forms. Brown shall notify the Court of any changes to her address within seven (7) days of the change by filing a notice of change of address.
