Accordingly, it is ORDERED: The clerk shall send Telamy: (1) a civil rights complaint form for use by prisoners in actions under 42 U.S.C. 1983; and (2) a complete set of forms needed to file a motion to proceed in forma pauperis (for prisoners). This case number should be written on both forms, and Amended Complaint should be written on the complaint form. Within twenty-one (21) days from the date of this Order, Telamy shall file an amended complaint that complies with the instructions set forth on the provided complaint form. The amended complaint must be typed or clearly written.
