ORDER GRANTING LEAVE TO PROCEED IN FORMA PAUPERIS. Plaintiff's motion to proceed in forma pauperis (Doc. #5 ) is GRANTED to the extent Plaintiff may proceed with this case without prepaying the entire filing fee. ORDER. Plaintiff's motion to proceed in forma pauperis (Doc. #2 ) is DENIED without prejudice. The Clerk of Court is directed to send Plaintiff a motion to proceed in forma pauperis and a Prisoner Consent Form and Financial Certificate approved for use in the Northern District.
