ORDER GRANTING LEAVE TO PROCEED IN FORMA PAUPERIS. The in forma pauperis motion, ECF No. #2 , is GRANTED to the extent Plaintiff is not required to prepay the $350.00 filing fee. Plaintiff is assessed an initial partial filing fee of $5.00 based on the monthly deposits in Plaintiff's inmate Trust Fund Account for the preceding six months as required by 28 U.S.C. 1915(b)(1)(A). Plaintiff has until 5/22/2020, to submit the initial partial filing fee. The Clerk of Court shall mail this Order with an appropriate cover letter to: Department of Corrections, Office of the General Counsel, 501 South Calhoun Street, Tallahassee, Florida 32399-2500. The Clerk of Court shall provide Plaintiff with a copy of the Prisoner Consent Form (included within a prisoner in forma pauperis application packet). Plaintiff must sign the Prisoner Consent Form and return it to the Court no later than 5/22/2020, as a supplement to his in forma pauperis motion.
