The clerk shall also send Plaintiff three AO 440 (Summons in a Civil Action) forms and three (3) USM-285 forms. Plaintiff shall have thirty days from the date of this order to file an amended complaint, provide three service copies of the amended complaint, along with the prepared summonses, and to file a motion to proceed in forma pauperis. Upon receipt of these materials, the clerk is directed to issue the summonses submitted by Plaintiff and send them, along with three certified copies of this order, the service copies of the amended complaint, and the prepared USM-285 forms to the United States Marshals Service.
