REPORT AND RECOMMENDATION. It is respectfully RECOMMENDED that Plaintiff's motion for leave toproceed in forma pauperis, ECF No. #5 , be DENIED pursuant to 28 U.S.C. 1915(g). It is further RECOMMENDED that this case be DISMISSED without prejudice to Plaintiff's refiling an action if he simultaneously submits the $402.00 filing fee because he failed to honestly disclose his prior cases. It is also RECOMMENDED that the Order adopting this Report and Recommendation direct the Clerk of Court to note on the docket that this cause was dismissed pursuant to 28 U.S.C. 1915(g). ORDER. The Clerk of Court shall forward to Plaintiff an application packet for requesting leave to proceed in forma pauperis and a 1983 civil rights complaint form. Plaintiff has until 8/19/2021, to either: (1) file a motion requesting leave to proceed in forma pauperis, supported by a computer printout of the transactions in Plaintiffs inmate bank account for the 6- month period of time preceding the initiation of this case, or (2) pay the full amount of the filing fee ($402.00 if paid in full).
