ORDER. Plaintiffs request to proceed in forma pauperis, as articulated in the complaint (ECF No. #1 ), is DENIED without prejudice, because it is not submitted on the proper IFP form and Plaintiff did not provide his inmate account statement detailing the last six months of transactions to verify eligibility. The Clerk of Court shall provide Plaintiff with a civil rights complaint form and an in forma pauperis application packet for use by a prisoner.Plaintiff shall have until June 9, 2022, to file the in forma pauperis application (along with the inmate account statement), or alternatively, pay the $402 filing fee and to file the amended complaint, on the proper form, if Plaintiff wants to continue with this case. If Plaintiff no longer seeks to proceed with this case, he should file a notice of voluntary dismissal pursuant to Fed. R. Civ. P. 41(a) by June 9, 2022.
