ORDER: The Clerk of Court shall mail to Plaintiff another copy of the first Order entered, ECF No. #3 , and the second Order entered, ECF No. #5 . Mail to Plaintiff shall be addressed: James Lee Ballard - Inmate; #ASO13MNI009026; Alachua County Jail, 333 NE 39th Avenue, Gainesville, Florida 32609. Plaintiff shall have until 7/27/2022, to comply with the first Order, ECF No. #3 . The Clerk of Court shall provide Plaintiff with another in forma pauperis application packet for use by prisoners and detainees. The Clerk of Court shall provide Plaintiff with another civil rights complaint form for use by non-prisoners. The Clerk of Court shall return this file upon receipt of Plaintiff's response to this Order or no later than 7/27/2022. ( Notify Chambers on 7/27/2022. Amended Pleadings due by 7/27/2022. The Clerk of Court shall re-mail the prior Order, ECF No. #3 , to Plaintiff along with this Order, ensuring the mailing label states Plaintiff's name as: James Lee Ballard. The Clerk of Court shall also forward to Plaintiff an application packet for requesting leave to proceed in forma pauperis by a non-prisoner. Plaintiff has until 7/1/2022, to either: (1) file a motion requesting leave to proceed in forma pauperis, supported by a financial affidavit, or (2) pay the full amount of the filing fee ($402.00 if paid in full).
