ORDER: Plaintiff's Amended Complaint, ECF No. #11 , is STRICKEN for failure to comply with the Court's prior order and with the Rules governing these proceedings and will not be considered. The Clerk is directed to STRIKE ECF No. #11 . Plaintiff's Motion for an Extension of Time, ECF No. #13 , is DENIED, in part. Plaintiff has 21 days to comply with the Court's orders. The Clerk of Court shall provide Plaintiff with a civil rights complaint form and an in forma pauperis application packet for use by a prisoner. Plaintiff shall have until August 18, 2022, to file an amended in forma pauperis application, signed by an Authorized Penal Official, or alternatively, pay the $402 filing fee. Plaintiff must complete a new civil rights complaint form by August 18, 2022, marking it as "second amended complaint" and "Case No. 4:22-cv-00211-MW-MAF." If Plaintiff no longer seeks to proceed with this case, he should file a notice of voluntary dismissal pursuant to Fed. R. Civ. P. 41(a) by August 18, 2022.
