ORDER. Plaintiff's motion to proceed in forma pauperis is DENIED WITHOUT PREJUDICE because (1) it is not on the proper form and (2) it is unclear whether Plaintiff is eligible for IFP status unless he meets the imminent danger exception and meets the financial criteria. Plaintiff's motion to introduce documents, ECF No. #4 , is DENIED WITHOUT PREJUDICE. The Clerk of Court shall provide Plaintiff with a civil rights complaint form and an in forma pauperis application packet for use by a prisoner. If Plaintiff wishes to proceed with this action, he must complete an amended complaint on the proper form by January 17, 2023. No later than January 17, 2023, Plaintiff shall complete an in forma pauperis application packet for use by prisoners or, alternatively pay the $402 filing The Clerk of Court shall return this case upon Plaintiff's compliance with this Order or no later than January 17, 2023.
