ORDER - The Clerk of Court shall forward to Plaintiff an application packet for requesting leave to proceed in forma pauperis by state prisoners. Plaintiff has until 3/9/2023, to either: (1) file an in forma pauperis motion (which must include a prison account statement for the 6-month period of time preceding the filing of this case), or (2) pay the full amount ($402.00) of the filing fee. The Clerk of Court shall also provide Plaintiff with a civil rights complaint form for use by state prisoners.
