ORDER GRANTING LEAVE TO PROCEED IN FORMA PAUPERIS - Plaintiff's in forma pauperis motion, ECF No. #5 , is GRANTED to the extent Plaintiff is not required to prepay the filing fee. Plaintiff is assessed an initial partial filing fee of $4.15 based on the monthly deposits in Plaintiffs Inmate Trust Fund Account for the preceding six months as required by 28 U.S.C. 1915(b)(1)(A). The filing fee is reduced to $350.00. Plaintiff has until 10/7/2024, to submit the initial partial filing fee. The Clerk of Court shall mail this Order with an appropriate cover letter to: Department of Corrections, Office of the General Counsel, 501 South Calhoun Street, Tallahassee, Florida 32399-2500. ORDER - Plaintiff's motion for leave to proceed in forma pauperis, ECF No. #2 , is DENIED. The Clerk of Court shall forward to Plaintiff an application packet for requesting leave to proceed in forma pauperis by state prisoners. Plaintiff has until 9/24/2024, to file another in forma pauperis motion supported by a printout of his inmate bank account for the 6-month period of time prior to case initiation along with a signed prisoner consent form.
