September 11, 2019 Opinion or Order Filing 4 ORDER. Plaintiff's motion to proceed in forma pauperis (Doc. #2 ) is DENIED without prejudice. The clerk of the court is directed to send to Plaintiff: (a) a civil rights complaint form for use by prisoners in actions under 42 U.S.C. 1983; (b) a motion to proceed in forma pauperis for use by prisoners; and (c) a Prisoner Consent Form and Financial Certificate approved for use in the Northern District of Florida. On or before 10/11/2019 Plaintiff shall either: (a) file an amended civil rights complaint, which must be typed or clearly written, submitted on the court approved form, and titled "Amended Complaint; or (b) file a notice of voluntary dismissal. On or before 10/11/2019 Plaintiff also shall either: (a) pay the $400.00 ($350.00 filing fee and $50.00 administrative fee); or (b) submit a fully completed motion to proceed in forma pauperis with the requisite Prisoner Consent Form and Financial Certificate signed by a prison or jail official, and an attached computer printout of the transactions in Plaintiff's inmate trust account during the six-month period of incarceration preceding the filing of this suit.
