ORDER. Plaintiff's motion for leave to proceed in forma pauperis, Doc. #6 , is GRANTED to the extent that this case may proceed without prepayment of the entire filing fee. The clerk of court shall assess Plaintiff $80.00 as an initial partial filing fee as provided in 28 U.S.C. 1915(b)(1)(A). Plaintiff shall have 30 DAYS from the date this order is docketed in which to submit the initial partial filing fee as assessed by the clerk of court. The clerk of court is directed to MAIL a copy of this order, with the appropriate cover letter to: Department of Corrections, 501 South Calhoun St., Tallahassee, FL 32399-2500, Attention: Agency Clerk. No further action shall be taken with regard to Plaintiffs claims until the initial partial filing fee is received. Any party who desires to consent to trial of this case by a Magistrate Judge should execute the attached consent form and return it to the clerk of the court. The clerk will retain any such form and will refer the consent form to the district judge only if all parties consent. (Fee due by 6/20/2022.) (Order and letter mailed to facility.)
