Filing 4 ORDER denying without prejudice #2 Motion for Leave to Proceed in forma pauperis. The clerk of the court shall send to Plaintiff: a. a complaint form for use by prisoners in civil rights actions filed under 42 U.S.C. 1983; and b. an application packet for leave to proceed in forma pauperis for use by prisoners. On or before 10/6/2022, Plaintiff shall file: a. an amended complaint which must be typed or clearly written, submitted on the court-approved form, and titled "Amended Complaint"; or b. a notice of voluntary dismissal pursuant to Rule 41 of the Federal Rules of Civil Procedure. On or before 10/6/2022, Plaintiff shall: a. submit payment of $402 ($350 filing fee and $52 administrative fee) to the clerk of the court; or b. a complete application to proceed in forma pauperis. This includes the motion and financial affidavit on the court approved forms, the Prisoner Consent Form and Financial Certificate, and a copy of his inmate trust account for the six-month period immediately preceding the commencement of this suit.
