ORDER. Plaintiff's motion to proceed in forma pauperis, Doc. #3 , is DENIED without prejudice. The clerk of the court is directed to send Plaintiff: (a) a motion to proceed in forma pauperis and financial affidavit for use by prisoners; and (b) a Prisoner Consent Form and Financial Certificate approved for use in the Northern District of Florida. If Plaintiff wishes to proceed with this action, on or before January 3, 2023, Plaintiff shall (a) pay the $402.00 fee ($350 filing fee and $52 administrative fee) to the clerk of the court; or (b) submit a fully completed application for leave to proceed in forma pauperis, including (1) the motion to proceed in forma pauperis and financial affidavit on the court approved forms (2) a Prisoner Consent Form and Financial Certificate signed by a prison official, and (3) a computer printout of the transactions in Plaintiffs inmate trust account during the six- month period of incarceration preceding the filing of this suit.
