November 26, 2018 Opinion or Order Filing 8 PAPERLESS ORDER denying #6 Motion for Leave to Proceed in forma pauperis. Plaintiff has filed a purported amended motion for leave to proceed in forma pauperis ("IFP"). However, in contravention of the court's order of November 13, 2018, plaintiff did not file the amended IFP motion on the court-provided form, which the court mailed to petitioner. Furthermore, plaintiff has not submitted the statutorily required prison trust statement, which must fully cover the 6-month period immediately preceding the filing of the complaint. 28 U.S.C. 1915(a)(2). Therefore, the court denies plaintiff's amended IFP motion without prejudice. The court's order of November 13, 2018, DE#4, remains in full force. As explained in said order, if plaintiff does not pay the $350 filing fee, his new IFP motion must be filed (i.e., received by the court and docketed) by December 10, 2018.
