ORDER DENYING APPLICATION TO PROCEED IN FORMA PAUPERIS BY A PRISONER re #3 - Signed by JUDGE JILL A. OTAKE on 5/26/2022. Eblacas is DIRECTED to submit a complete Application to Proceed In Forma Pauperis by a Prisoner that includes: (1) a certificate completed by a prison official; (2) an account statement for the six months immediately preceding the commencement of this action showing all deposits and withdrawals to Eblacas's account during that period; and (3) Eblacas's consent to the withdrawal of funds from his account.
