Filing 14 MERIT REVIEW ORDER entered by Chief Judge James E. Shadid on 11/9/2018. IT IS THEREFORE ORDERED: 1) Plaintiff's complaint is dismissed as a violation of Rule 8 of the Federal Rules of Civil Procedure. 2) Plaintiff must file an amended complaint in compliance with this order within 21 days or on or before November 30, 2018. If Plaintiff fails to file an amended complaint by the November 30, 2018 deadline or fails to follow the directions provided by the Court, his case will be dismissed. 3) The Clerk of the Court is to provide Plaintiff with a blank complaint form. To avoid additional confusion, Plaintiff should use the provided form for his amended complaint. 4) The Clerk of the Court is to reset the internal merit review deadline within 21 days of this order. 5) Plaintiff is reminded he must immediately notify the Clerk of the Court in writing of any change in his mailing address or phone number. Failure to provide this information could lead to the dismissal of Plaintiff's lawsuit. 6) Plaintiff's motion for a status update is denied as moot. #12 . TEXT ORDER Entered by Chief Judge James E. Shadid on 11/7/18: Plaintiff has filed a complaint which the Court must review pursuant to 28 U.S.C. 1915A. However, Plaintiff has also filed two documents entitled "Declaration" in which he complains about continued problems at his correctional center. #11 , #13 . Plaintiff must first address his concerns through the grievance procedure at his correctional center. However, in his most recent filing, Plaintiff states he intends to kill himself if he does not receive the help he needs. #13 . Although the Defendants have not yet been served, the Court will order the Illinois Attorney General's Office to immediately provide a copy of Plaintiff's declaration #13 directly to the mental health department at Plaintiff's facility and file a notice of compliance within seven days of this order. Filing 10 TEXT ORDER entered by Chief Judge James E. Shadid on 10/4/2018. On September 21, 2018, Plaintiff, a pro se prisoner, filed a complaint and a motion to proceed IFP. Plaintiffs handwritten IFP application did not contain the information required of all litigants. Therefore, on September 27, 2018, the Court denied Plaintiffs IFP motion and directed him to fill out the complete IFP application with a financial affidavit. Plaintiff was also directed to indicate when he received the $5000 settlement Plaintiff mentioned. Plaintiff was given 14 days to file his new IFP motion or his case would be dismissed. Plaintiff has instead submitted a letter to the Court which has been filed as a motion for change of judge. #8 Plaintiff asks to confirm that the Court received the appropriate number of pages with his complaint. In addition, Plaintiff has attached a declaration stating he "would like to have the chance to change judges." (Mot., p. 3). Plaintiff has provided no basis for his request and the motion is denied. #8 . The Clerk of the Court is to provide Plaintiff with a copy of the docket sheet. TEXT ORDER granting #9 Petition to Proceed In Forma Pauperis. Pursuant to 28 U.S.C. Section 1915(b)(1), Plaintiff is assessed an initial partial filing fee of $3.95. The agency having custody of Plaintiff is directed to forward the initial partial filing fee from Plaintiff's account to the Clerk of Court. After payment of the initial partial filing fee (or immediately if no funds are available for that payment) the agency having custody of Plaintiff is directed to make monthly payments of 20 percent of the preceding month's income credited to Plaintiff's account to the Clerk of Court. Income includes all deposits from any source. The agency having custody of the plaintiff shall forward these payments each time Plaintiff's account exceeds $10, until the filing fee of $350 is paid in full.
