TEXT ORDER: Summons was previously issued for Defendants Burton and the Woodford County Sheriff on 2/17/2023. Counsel for Defendant Burton entered their appearances in this case on 3/3/2023. On 3/6/2023, the Clerk issued summons to Defendants Woodford County #7 , Woodford County Sheriffs Department #8 , and Burton #9 . None of those three summonses included the Plaintiff's name and address (nor did the original summons to Defendant Woodford County Sheriff). Fed. R. Civ. P. 4(b) provides, in relevant part, that "the plaintiff may present a summons to the clerk for signature and seal. If the summons is properly completed, the clerk must sign, seal, and issue it to the plaintiff for service on the defendant." (emphasis added). A summons must state the name and address of an unrepresented plaintiff. Fed. R. Civ. P. 4(a)(1)(C). In light of all this, the three summonses #7 , #8 , #9 issued on 3/6/2023 are STRICKEN. The Clerk is directed to re-issue only the summonses for Defendants Woodford County Sheriff and Woodford County with the Plaintiff's name and address included. Entered by Magistrate Judge Jonathan E.
