Petitioner has filed a Motion to Proceed in Forma Pauperis #2 , indicating that he cannot pay for the cost of this litigation and requesting the appointment of counsel. Petitioner did not, however, include the prisoner trust fund ledger as required under 28 U.S.C. 1915(a)(2), nor did he substantiate any efforts to obtain counsel without intervention of the court.
