TEXT ORDER entered by Magistrate Judge Eric I. Long on 6/29/2026. Pursuant to this Court's Text Order 3 entered on 6/9/26, Plaintiff has re-filed an Application to Proceed in District Court Without Prepaying Fees or Costs #4 . In addition, Plaintiff has submitted a summons and USM-285 for many defendants in this case. However, Plaintiff did not submit a summons and USM-285 form for Defendant City of Danville Illinois and Defendant Danville Police Department. For Plaintiff's Complaint and Application to be considered, Plaintiff must submit a summons and USM-285 form for each named defendant being sued. If Plaintiff does not intend to name the City of Danville Illinois or the Danville Police Department as defendants, Plaintiff must file an amended complaint that does not list then as named defendants. Within 21 days, Plaintiff is directed to either fill out a summons and USM-285 form for each remaining defendant being sued or file an amended complaint. Failure to do will result in dismissal of this case without prejudice. (FDS)

TEXT ORDER entered by Magistrate Judge Eric I. Long on 6/9/2026. Plaintiff has submitted a complaint. In addition, Plaintiff has filed an Application to Proceed in District Court Without Prepaying Fees or Costs (Short Form) #2 , which is hereby DENIED with leave to refile. If Plaintiff does refile her application, he must complete in its entirety and submit the long form Application to Proceed Without Prepaying Fees or Costs, not the short form Application. In addition, for Plaintiff's complaint and application to be considered, Plaintiff must supply the Court with completed summons and U.S. Marshals service forms (USM-285). Plaintiff has not yet submitted the summons or USM-285 forms. Therefore, within 21 days, Plaintiff is directed to fill out a summons and USM-285 forms for each defendant being sued. Failure of Plaintiff to complete and return the forms within 21 days will result in summary dismissal of the complaint, without prejudice. In addition, within 21 days, Plaintiff is directed to either refile his Application to Proceed Without Prepaying Fees or Costs or pay the filing fee. Failure to do so will result in dismissal of this case without prejudice. The Clerk is directed to provide Plaintiff with copies of the forms and long form application. (JS)
