TEXT ORDER granting #8 Petition to Proceed In Forma Pauperis. Pursuant to 28 U.S.C. Section 1915(b)(1), Plaintiff is assessed an initial partial filing fee of $55.00. The agency having custody of Plaintiff is directed to forward the initial partial filing fee from Plaintiff's account to the Clerk of Court. After payment of the initial partial filing fee (or immediately if no funds are available for that payment) the agency having custody of Plaintiff is directed to make monthly payments of 20 percent of the preceding month's income credited to Plaintiff's account to the Clerk of Court. Income includes all deposits from any source. The agency having custody of the plaintiff shall forward these payments each time Plaintiff's account exceeds $10, until the filing fee of $350 is paid in full. The Clerk is directed to mail a copy of this order to Plaintiff's place of confinement, to the attention of the Trust Fund Office. Entered by Judge James E. Shadid on 2/4/2020. (SAG, ilcd)

TEXT ORDER entered by Judge James E. Shadid on 1/27/2020. Plaintiff has filed a motion to proceed in forma pauperis #3 along with a motion for an order directing the Sangamon County Jail to send Plaintiff's Trust Fund Ledgers to the Court #5 . The motion for an order is denied as moot #5 . The Clerk has obtained copies of Plaintiff's ledgers. However, Plaintiff has not adequately filled out the IFP motion. Plaintiff must list any income he receives from any source including income from gifts. Plaintiff has listed zeros, but his Trust Fund ledgers indicate he has received $550 since December 4, 2019. Plaintiff is reminded any motion filed with the Court must contain accurate, truthful information. Therefore, Plaintiff's motion to proceed IFP is denied #3 . Plaintiff must either pay the $400 filing fee in full or file a new motion to proceed IFP with accurate information within 14 days or this order. Plaintiff must list any income from any source and must clearly indicate if he has any outside accounts or income prior to his arrest. Plaintiff must also fill out the entire, written IFP form. If Plaintiff fails to pay the $400 filing fee or file a new IFP motion with accurate information on or before February 10, 2020 his case will be dismissed without prejudice.(SAG, ilcd)
