TEXT ORDER entered by Judge James E. Shadid on 8/14/2020. Plaintiff has filed a motion for leave to proceed in forma pauperis (IFP), but the form is blank. In addition, Plaintiff has not provided the required copies of his Trust Fund Ledgers for the six months prior to filing his complaint. See 28 U.S.C. 1915(a)(2). Therefore, Plaintiff's motion is denied #3 . The Clerk of the Court is directed to provide Plaintiff with an IFP form. If Plaintiff wishes to proceed IFP he must fully complete this form and indicate if he received money from any source including gifts from family or friends over the last six months. Plaintiff must either file his new IFP motion in compliance with this order, or pay the $400 filing fee with 21 days or on or before September 4, 2020. If Plaintiff does not take any action, his lawsuit may be dismissed. (SAG, ilcd) |
