TEXT ORDER by U.S. Magistrate Judge Tom Schanzle-Haskins. Before the Court is pro se Plaintiff's "Notice" #3 filed 1/29/2021 in response to the Court's Text Order of 1/7/2021 directing that Plaintiff file a signed complaint with her current email address. The Plaintiff's "Notice" filed a copy of her Complaint with a Certificate of Service which included her signature and email address. The Court deems this sufficient compliance with it Text Order of 1/7/2021 and Fed. R. Civ. P. 11(a). The Clerk is directed to docket Plaintiff's "Notice" #3 as her Amended Complaint. Pro se Plaintiff's Application to Proceed in District Court Without Prepaying Fees or Costs #2 is ALLOWED. The Clerk of the Court is directed to send notice of lawsuit waiver of service forms to the Defendant at the address provided by Plaintiff along with a copy of the Complaint, two copies of the waiver form, and a prepaid envelope to return the form by first class mail to the Clerk. If a Defendant fails to sign and return a waiver within 30 days from the date said forms were sent, the Court will direct personal service by the U.S. Marshal at the Defendant's expense. Plaintiff is ordered to file by 1/31/2021 a signed copy of the Complaint and include her e-mail address if she has one.
