December 7, 2022 Opinion or Order TEXT ORDER entered by Judge James E. Shadid on 12/7/2022. Plaintiff has signed his motion to proceed IFP, but he left the remainder of the form blank. Plaintiff is required to disclose all of his assets to the Court if he wishes to proceed IFP. Therefore, he must complete the IFP from and indicate any money he has received including gifts from family or friends. Plaintiff must also indicate if he has any outside accounts. In addition, Plaintiff must provide a copy of his jail trust-fund account for the six-month period immediately preceding the filing of his complaint. See 28 U.S.C. 1915(a)(2). Plaintiff's IFP motion is therefore denied #3 . Within 21 days, Plaintiff must either pay the $402 filing fee in full or file a complete motion for leave to proceed IFP. If Plaintiff fails to either pay the fee or renew his IFP motion on or before December 29, 2022, his case will be dismissed. The Clerk is to provide Plaintiff with an IFP form to assist him.(SAG)
