TEXT ORDER entered by Judge Sue Myerscough on 10/8/2024. Plaintiff's petition to proceed in forma pauperis (d/e 2) is GRANTED. The Clerk is directed to provide Plaintiff with two forms: a Notice of a Lawsuit and Request to Waive Service of a Summons form and a Waiver of the Service of Summons form. Plaintiff is directed to send Defendant via certified mail a request to waive service of a summons in this action. In her request to waive service of a summons in this action, Plaintiff shall include two copies of the waiver form, a prepaid means of returning one signed copy of the waiver form, and a copy of Plaintiff's September 13, 2024 Motion for a Preliminary Injunction (d/e 1). Plaintiff shall consult Federal Rule of Civil Procedure 4(d) on Waiving Service. Plaintiff has not provided proof of service as to any of the Defendants since filing her motion on September 13, 2024. Plaintiff is directed to pay the filing fee of $405 on or before October 4, 2024, or to submit a Petition to Proceed in Forma Pauperis pursuant to Local Rule 5.4(c). The Clerk is DIRECTED to send a physical copy of this order to the Plaintiff at the address listed on the docket and to provide a copy of the Application to Proceed in District Court without Prepaying Fees or Costs.
