TEXT ORDER entered by Judge Colleen R. Lawless on 10/24/2025. Plaintiff, who resides in Northlake, Illinois, has filed a Complaint #1 , a Petition to Proceed in forma pauperis (IFP), a Motion for Counsel #4 , and a Motion for Injunctive Relief #5 . The Clerk of the Court is DIRECTED to send Plaintiff the Court's Long Form IFP application. Plaintiff has thirty days from the entry of the Court's Order to pay the $405 filing fee or Petition the Court to proceed IFP using the provided long form. The Court informs Plaintiff that any material misrepresentations of his financial status will result in the dismissal of his case with prejudice. Accordingly, the Court strikes Plaintiff's short form IFP Petition #3 . Plaintiff's Motion for Injunctive Relief #5 is MOOT. See Maddox v. Love, 655 F.3d 709, 716 (7th Cir. 2011) ("[The plaintiff's] prayers for injunctive relief are moot because he is no longer an inmate at [the facility]... [and] has not shown a realistic possibility that he will again be incarcerated in the same state facility...."). (KE)
