TEXT ORDER entered by Judge Jonathan E. Hawley on 2/25/2025. On January 31, 2025, the Court directed Plaintiff to show good cause why this case should not be dismissed for failure to keep the Court informed of his current address after mail the Court sent to Plaintiff was returned as undeliverable. (d/e 1/31/2025). The Court warned Plaintiff that failure to show good cause within 14 days would result in the dismissal of this case. Id. A copy of the Order was mailed to Plaintiff's last known address, but it was returned to the Court as undeliverable on February 18, 2025. (Doc. 8). Pursuant to Central District of Illinois Local Rule 16.3(K), every pro se plaintiff must notify the Clerk of this Court in writing of any change of address during the entire pendency of his case. It is not the Court's responsibility to independently maintain current addresses on all parties to pending actions. To date, Plaintiff has not notified the Court of a change of address. Therefore, Plaintiff's Complaint is DISMISSED WITH PREJUDICE for failure to prosecute. Fed. R. Civ. P. 41(b). Plaintiff's Motion to Request Counsel #4 is DENIED. Plaintiff remains responsible for the remainder of the outstanding filing fee even though his case has been dismissed. (d/e 1/22/2025). If Plaintiff wishes to appeal this dismissal, he may file a notice of appeal with this Court within 30 days of the entry of judgment. Fed. R. App. P. 4(a). A motion for leave to appeal in forma pauperis should set forth the issues Plaintiff plans to present on appeal. Fed. R. App. P. 24(a)(1)(C). If Plaintiff chooses to appeal, he will be liable for the $605 appellate filing fee irrespective of the outcome of the appeal.(ANW)

TEXT ORDER entered by District Judge Jonathan E Hawley on 1/22/25 granting #3 Petition to Proceed In Forma Pauperis. Plaintiff's trust fund ledgers indicate he has no funds to make an initial partial payment of the filing fee. Pursuant to 28 U.S.C. Section 1915(b) (4), the initial partial filing fee is waived. The agency having custody of Plaintiff is directed to make monthly payments of 20 percent of the preceding month's income credited to Plaintiff's account to the Clerk of Court. Income includes all deposits from any source. The agency having custody of the plaintiff shall forward these payments each time Plaintiff's account exceeds $10, until the filing fee of $350 is paid in full. The Clerk is directed to mail a copy of this order to Plaintiff's place of confinement, to the attention of the Trust Fund Office. (ANW)
