January 6, 2026 Opinion or Order TEXT ORDER: Plaintiff's #2 Motion for Leave to Proceed in forma pauperis is DENIED without prejudice for two reasons. First, the caption of Plaintiff's motion reads "United States District Court for the District of Colorado," rather than this Court. Second, and more importantly, Plaintiff's motion contains several inconsistencies. For example, page 2 of Plaintiff's motion indicates that Plaintiff receives $298.00 per month in the form of public assistance, but $0.00 per month in "total monthly income." On page five, Plaintiff's age is listed as "Ss," rather than a number. And finally, Plaintiff's signature on the last page is difficult to read. Accordingly, Plaintiff may refile a motion to proceed IFP using the proper form, which is entitled, "AO-239 Application to Proceed in District Court Without Prepaying Fees or Costs (Long Form in forma pauperis)" and may be found under the "Forms" tab of this Court's website.
