Filing 19 WRITTEN Opinion entered by the Honorable Charles P. Kocoras on 8/13/2012: The Order filed on August 6, 2012 (Doc [ 17 ]) is vacated, and is replaced with this Amended Order. The Court reinstates this case. Plaintiff's current motions for le ave to file this suit in forma pauperis ("IFP") (Docs [ 13 ] & [ 15 ]) is granted. Plaintiff may proceed with his amended complaint, and the Clerk shall issue summonses for service of the amended complaint (Doc [ 11 ]) on Superintendent B ryant, Dr. Kahn, Nurse Judy Price, and Officer Gansel. The U.S. Marshal is appointed to serve these Defendants. The other Defendants are dismissed. Plaintiff's motion for the appointment of counsel (Doc [ 16 ]) is denied without prejudice. Filing 4 WRITTEN Opinion entered by the Honorable Charles P. Kocoras on 1/18/2012: Plaintiff's motion for leave to file this suit in forma pauperis ("IFP") (Doc [ 3 ]) is denied without prejudice. Plaintiff's complaint is dismissed without prejudice. To proceed with this case, Plaintiff must, within 30 days of the date of this Order, 1) submit a completed IFP application or prepay the $350 filing fee, and 2) file an amended complaint. The clerk is directed to mail to Plaintiff an IFP application and an amended civil rights complaint form. The clerk shall also mail a copy of this Order to the Cook County Department of Corrections Sheriff's Office so that, if Plaintiff is in need of current medical attention, jail officials can ensure that such attention is provided.
