Plaintiff's complaint is dismissed without prejudice. Plaintiff is g iven until 3/16/12 to: (1) either submit a completed in forma pauperis application or prepay the $350 filing fee, and (2) submit an amended complaint in accordance with this order. The clerk shall send Plaintiff an in forma pauperis application and an amended complaint form, along with a copy of this order. If the court receives no pleadings in response to this order by 3/16/12, this case will be summarily dismissed. Plaintiffs motion for the appointment of counsel 5 is denied without prejudice.
