WRITTEN Opinion entered by the Honorable George W. Lindberg on 6/13/2012: Plaintiff's motion for leave to file in forma pauperis (Dkt. No. 3 ), is granted. The Court orders the trust fund office at plaintiff's current place of incarceration to deduct $10:05 from plaintiff's account for payment to the Clerk of Court as an initial partial filing fee. The clerk shall send a copy of this order to the Supervisor of Inmate Trust Fund Accounts, Cook County Dept. of Corrections Admin istrative Office, Division V, 2700 S. California, Chicago, Illinois 60608. Sheriff Tom Dart is added as a defendant solely so that plaintiff may discover the identity of John Doe defendants. The clerk shall issue summonses for service on all defendan ts and send the plaintiff a Magistrate Judge Consent Form and Instructions for Submitting Documents along with a copy of this order. The United States Marshals Service is appointed to serve all defendants.
