Plaintiff's complaint is dismissed without prejudice to Plaintiff submitting an amended complain t in accordance with this order within 30 days of the date of this order. The clerk shall send Plaintiff an amended complaint form. Plaintiff's failure to comply timely with this order will result in the dismissal of this case.
