WRITTEN Opinion entered by the Honorable John J. Tharp, Jr on 7/26/2012: Plaintiff's motion for leave to proceed in forma pauperis (Dkt. No. 3), is granted. The Court authorizes and orders Cook County Jail officials to deduct $20.00 fr om the plaintiff's account, and to continue making monthly deductions in accordance with this order. The clerk shall send a copy of this order to the Supervisor of Inmate Trust Fund Accounts, Cook County Dept. of Corrections Administrative Of fice, Division V, 2700 S. California, Chicago, Illinois 60608. Defendant Cook County is dismissed. The clerk shall issue summonses for service on defendant Sheriff Dart and Aramark Food Services and send the plaintiff a Magistrate Judge Consent For m and Instructions for Submitting Documents along with a copy of this order. The United States Marshals Service is appointed to serve all defendants. Plaintiff may not proceed with claims against Sheriff Dart and remains in the case solely so that plaintiff can identify the John Doe defendants. Mailed notice(vcf, )
