ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice and his complaint #1 is dismissed without prejudice. If Plaintiff wants to pursue this litigation, he must submit a renewed in forma pauperis application that (1) is signed, (2) is fully and accurately completed, (3) is certified by an authorized correctional official, and (4) attaches a copy of his trust fund statement showing activity in his account for the six-month period immediately before he filed this suit. Alternatively, he may pay the full statutory filing fee of $400. He must also submit an amended complaint that is signed. His failure to comply by October 18, 2018, will result in summary dismissal of this case. The Clerk is directed to send a blank in forma pauperis application, a blank amended complaint form, and a copy of this order to Plaintiff. [see STATEMENT] Signed by the Honorable Frederick J.
