Filing 8 ORDER: The Court denies Plaintiff Joshua Scott's application for leave to proceed in forma pauperis #5 without prejudice. If Scott wants to pursue this case, by September 28, 2018, he must either: (1) pay the full statutory filing fee of $400; or (2) submit a renewed application for leave to proceed in forma pauperis that (a) is certified by trust fund officers at all facilities where he was incarcerated for the six months prior to submission of his complaint (approximately February 3, 2018, to August 3, 2018) and (b) attaches copies of his trust fund statements showing activity in his accounts at those facilities for any portion of the six month look-back period. In addition, Scott must submit, also by September 28, 2018, an amended complaint using the Court's form, along with a completed USM-285 (Marshals service) form for each Defendant named in the amended complaint. If Scott fails to comply, the Court will summarily dismiss this case. Scott's request #3 for a copy of his original complaint is granted, to the extent that in this single instance, the Clerk will send him a copy of that pleading #1 . Scott's motion for attorney representation #4 is denied without prejudice, as it is too early to determine if counsel is appropriate. Scott is advised that he must promptly submit a change-of address notification if he is transferred to another facility, as the Clerk will make no further efforts to locate him. If he does not keep the Clerk apprised of his correct address, this case will be subject to dismissal. The Clerk is directed to: (1) send Scott an application for leave to proceed in forma pauperis, an amended complaint form, a blank USM-285 form and instructions, a copy of this order, and a copy of his complaint #1 ; and (2) update Scott's address on the docket to reflect that he is incarcerated at the Stateville Correctional Center's Northern Reception and Classification Center under prisoner number M-08457.
