ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $17.86 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. Cermak Health Services is dismissed as a Defendant. The Court directs the Clerk of Court to (1) electronically send a copy of this order to the Supervisor of the Inmate Trust Fund Accounts at the Cook County Jail; (2) file Plaintiff's complaint #1 ; (3) issue summons for service of the complaint on Defendant Dart, who remains solely as a nominal Defendant to aid in identifying the unknown Defendants; (4) send Plaintiff one blank USM-285 service form to be completed for Defendant Dart (without which Dart cannot be served); and (5) send Plaintiff an amended civil rights complaint form, a magistrate judge consent form, filing instructions, and a copy of this order. The U.S. Marshal is appointed to serve Defendant Dart. The Court defers ruling on Plaintiff's motion for attorney representation #4 pending further case development. Plaintiff is warned that he will need to amend his complaint to name the unknown John and Jane Doe Defendants by name or this lawsuit may be dismissed.
